We are active in the provision of a comprehensive range of social welfare services, catering the needs from cradle to seniors, including residential aged care, home ageing, settlement and health, vocational and training, disability services, child care and many more community-based services. We are seeking an enthusiastic and highly skilled talent to fill the above position in our organization.

The appointee of this position is to provide clinical services and support to the recipients of our Home Ageing Services. This person will be responsible for the provision of high-quality services in accordance with all Government requirements, service standards, as well as all policies and procedures, vision, mission, and goals of our organization.

• Provide all aspects of clinical services and support to recipients of our Home Ageing Services under the Commonwealth Home Support Program (CHSP), Home Care Packages (HCP) and Short Term Restorative Care (STRC) program;

• Develop care plans with reference to the life goal of service recipients and their care needs;

• Coordinate the services provided by primary and allied health professionals to meet the clinical needs of service recipients;

• Maintain and develop a strong network with stakeholders;

• Promote the home ageing services of our organization.
